If you are referring a patient for a consultation, please call our office, and we will schedule this appointment at your patients earliest convenience. Please print our order form and fax it to our office with your patient’s demographics. We will call you to schedule the appointment at the earliest available time. If there are any special needs, give us a call and we will do our best to accommodate you.
